According to its creators Call of Juarez: Bound in Blood is aimed to make the player experience the "best West ever created". The game derives from spaghetti-western genre: gunslingers, outlaws, duels, prison breaks, bank robberies, stagecoach chases and conflicts with Native Americans, similar to another western game, Gun. It features two characters, brothers Ray and Thomas. The game plot is set in the American Civil War period and presents adventures of two brothers across different parts of North America.

Before most missions, the player can choose between one of the two brothers, but sometimes only one is available. Though the game's Story Mode is fairly short, each brother has a different effect on the missions: often the player will run into split paths for either brother to take, or certain key points where one brother needs to do particular things while the other does something else. For example, Thomas can lasso on top buildings for a vantage point to cover Ray while he attacks/dynamites certain areas. One brother's story mode will be quite different from the other's. At first the game just moves from mission to mission but once the game opens, between certain missions there will be an open Free World where the player can take side-missions as a bounty hunter or hired gun, respectively for either attack or defence, and other kinds of side-missions. In each mission and each Free World there will be a trader who will sell the player certain weapons available at different qualities.

In each Free World area there is a small town, a trader and out in the wilderness the player can find ruined buildings, farms, camps and bandits that can be occupying these. Bandits will also hide at certain road points to ambush people, attacking people on the roads and robbing caravans. It is up to the player whether or not to save the people getting attacked. Throughout the Free Worlds and in the mission areas there will be "Secret Content" most commonly found in chests that will unlock artwork or memories that can be viewed from the menu. Chests may also contain money, dynamite or ammo.

At the end of most missions the player will have a "Showdown" with the leader of the group the player is fighting. In Bounty Hunter missions the player needs to first defeat the leader in a gunfight before he will challenge the player to a final Showdown. During the Showdown the playercharacter will circle the opponent with his hand hovering over his gun waiting for the "bell" to ring before he draws. The player needs to try and keep his characters hand as close to the gun as possible by moving the analog/mouse in towards the gun, if moved too close too fast the character will suddenly close his hand then wave his index finger towards the screen as if he was saying "No, no.", this will also reset the distance between his hand and the gun. The player also need to keep the opponent lined up with the shot by moving in the opposite direction as he moves. The center of the shot will be just off the center of the screen on the right hand side. As soon as the bell rings the player has a very short period of time to pull the analog/mouse in towards the gun then take the shot when the cross hair lines up with his body/head. If the first shot is missed or drawn late the opponent will shoot the player character. As the player progresses in the game Showdowns get harder, opponents will circle more to throw the player's shot and draw faster.

The game begins in the middle of the story with Ray and Thomas pointing their guns at each other. William is lamenting on how things degenerated to the point where the two brothers became mortal enemies. The scene then shifts to the American Civil War where Ray is holding the line against Union soldiers at the Chattahoochee River, when he hears that the area Thomas is stationed is under attack. He then rushes to save his brother, but after they defeat the enemy they are ordered to retreat to Atlanta. They refuse because the line they are holding now is the last line of defense that protects their family's home. They desert their post to save their mother and younger brother William from the Union troops attacking their home. When they finally make it to their home, they realize their mother was killed and Ray promises to rebuild the house, promising that they will be a family again. Knowing their superior Colonel Barnsby will not let them off, they flee the town, bringing William with them.

The scene shifts to a year later. William enters a bar only to catch Thomas and Ray fighting. It is revealed that they have been kicked out of many towns for their unruly behavior and that Thomas had sex with the sheriff's daughter, instigating a duel between one of the McCalls and the sheriff where the latter is killed. In the aftermath, the whole town turns on the McCalls, forcing them to flee the country for Mexico, hoping to find the mythical Lost Treasure of Cortez in Mexico so they may use the money to rebuild their home. After escaping, Ray warns Thomas that if he ever steals another woman from him, he'll "take (his) life". While in a Mexican Saloon, a Hispanic woman named Marisa catches Ray's attention. She is then harassed by two men, compelling the McCalls to rescue her. However, it is later revealed that she is the mistress of Juan 'Juarez' Mendoza, the most notorious criminal in Mexico. Grateful, he invites them to dinner, and Ray accepts on their behalf with the intention of flirting with Marisa. While doing so, she only leads him on when he says he would kill Juarez for her.

After the meal, Juarez lets them in on a plan to find the treasure, promising a share of it to them. A man named Devlin is causing him trouble, already having hired a demolition crew to help him find the same gold. Juarez orders the McCalls to eliminate Devlin and his men, to which William objects to. After having fought through Devlin's men, Ray is about to shoot Devlin when William tries to stop him by preaching the word of God. Even then, Ray shoots Devlin anyway.

Meanwhile, an Apache tribe is discussing going to war with "the white people". Their leader, Running River, orders his son, Seeing Farther, to obtain over 300 rifles for their tribe. Seeing Farther is reluctant but does so to prove to his father that he is not like the whites, having blue eyes due to his mother being a caucasian. Around that time, the McCalls return to Juarez who orders them to meet up with his gun runner for those rifles as he needs them in exchange for a medallion currently in possession of the Apaches. The medallion is the key to finding the gold. Before the McCalls set off, Thomas confronts Marisa about Ray to which she reveals her love for him. Thomas reciprocates her feelings. At the meeting place, they find that the gun runner has been jailed after being captured by the Pinkertons. The McCalls proceed to break him out of jail and deliver him to Juarez. There, the gun runner discusses his terms with Juarez and reveals himself to be Colonel Barnsby, having worn bandages covering his whole head.

It isn't long before Barnsby reveals himself to the brothers. In what was supposed to be a meeting with Juarez at an abandoned mine, Barnsby and his men ambush the brothers and tie them up. William manages to free himself and subsequently frees Ray and Thomas. The brothers escape the mine and manage to catch up with Juarez and Marisa - in Comanche Territory. The Apaches eventually confront Juarez and reveal that they know he was trying to dupe them - the rifles he was planning to barter were useless. Seeing Farther reveals that he learned this from William, who is given a reward by the Indians. He chooses for their lives to be spared and the McCalls and Marisa are taken back to the village along with the horses belonging to Juarez. There, Seeing Farther reveals that he will help them find the medallion, explaining that his motivation for doing this is to prevent his tribe from going to war. Seeing Farther then goes with Ray and Thomas into Navajo territory and recover the medallion, not before tainting their sacred ground with bloodshed.

Upon their return, Seeing Farther formulates a plan to get Marisa and William out of the camp and asks Ray and Thomas to distract the Apache tribe while he teaches William how to use the Medallion, stating they would like to see a fight. When Ray confronts Thomas about the statement, he reveals his relationship with Marisa, sparking a feud between the two which shortly erupts into a fight, effectively distracting the tribe and tearing the brothers apart. The plan backfires, however, when Barnsby's army locates the brothers and destroys the Indian village leaving only the chief and his son (Seeing Farther) alive. Seeing Farther, William and Marisa are taken hostage and will only be returned upon trade of the medallion. Ray and Thomas proceed to rescue them, but Ray says that once they're finished, he never wants to see Thomas again. The brothers defeat Barnsby's men in a dusty ghost-town but by the time they reach Seeing Farther, he is already mortally wounded. Dying, he reveals that William and Marisa are still alive - in Juarez's hands. He also confesses to his father about his involvement in betraying the location of the medallion. Barnsby is left tied up - to the chief's mercy. The chief approaches him with a knife, but rather than killing him, cuts the ropes and sets him free. By this time, Running River proclaim he will not fight anymore. Then, he becomes Calm Water, the Apache hermit who helps Billy Candle in the original Call of Juarez.
William is being interrogated by Juarez for information on the use of the Medallion. He refuses to speak, instead chanting repeatedly, "My faith is my shield.", serving only to aggravate Juarez into testing that very faith. Having secretly observed Marisa passing William a gun, Juarez orders one of his men to stab the boy. William shoots the man in self-defense, appalled by his own actions. Still refusing to disclose anything, Juarez orders his men to kill him. Marisa pleads for William's life, revealing she is carrying Juarez's child but is unsuccessful. At the same time, Ray and Thomas infiltrate Juarez's camp separately, with Ray having to rescue William and Thomas having to prepare horses for their escape as well as rescuing Marisa. Thomas prepares the horses and frees Marisa who tells him that William's dead and convinces him to leave Ray behind and focus on the treasure, revealing she also knows how to use the Medallion. Meanwhile, Ray rescues William from Juarez's men and attempts an escape, only to find that Thomas abandoned them. They escape through an underground pathway only to be obstructed by Juarez. They engage in a duel and Ray emerges victorious, proceeding to exact revenge on Thomas.

The story returns to the present-time, where Ray has tracked Thomas and Marisa to the treasure vault. Ray faces off against Thomas - demanding justice. William moves between them and declares he won't allow Ray to shoot Thomas, threatening to "reach on the count of three". By three, Ray shoots William, realising he was only reaching for the Bible. William collapses and dies, his Bible landing at Barnsby's feet who remarks that it seems the treasure really is cursed. A trap is triggered, causing the chamber to fill with sand, forcing the McCalls to move to higher ground, fending off Barnsby's men as they proceed. Eventually, a duel ensues between Barnsby and the chosen McCall, resulting in the former killed. The McCalls, together with Marisa, decide to abandon the treasure, knowing it was cursed anyway. William's sacrifice compels Ray to becomes a priest and he carries William's Bible. He also weds Thomas and Marisa. In the closing artwork, it is implied that the family home in Georgia is rebuilt. Others instead, claim it is actually the town of Hope which became the home of Ray, Thomas, Marisa, and their son "Billy Candle" in the first game.
